To help make your first visit as smooth and efficient as possible, we will send you our New Patient Case History Form before arriving at the office.

Completing your intake form in advance will save you time during your first appointment and provide our doctors with important background information about your health and condition.

How to Prepare for Your First Visit To help you get the most out of your first chiropractic appointment, here are a few simple steps:

Complete Your Paperwork in Advance

Download, fill out, print/email the New Patient Case History Form before your visit. Bringing it with you will reduce wait time and allow us to focus more on your consultation and care.

Bring Important Information

Please remember to bring: A photo ID Your insurance card (if applicable) Any recent X-rays, MRIs, or medical reports related to your condition.

Dress Comfortably

Wear loose, comfortable clothing so the doctor can easily perform your examination and any necessary movement assessments.

Arrive a Few Minutes Early

Arriving 10 minutes before your appointment helps ensure a relaxed, stress-free start to your visit.

Write Down Your Questions

If you have specific concerns, goals, or questions, jot them down and bring them with you so we can address them during your consultation.

Your First Visit – What to Expect

Once your initial paperwork is complete, you’ll meet with the doctor for a one-on-one consultation to discuss your health concerns, current symptoms, and wellness goals. During this time, the doctor will ask detailed questions and perform a comprehensive examination, which may include specialized tests related to your condition.

In most cases, patients will receive their first chiropractic treatment during their second visit.

Before you leave, you’ll also receive personalized home care instructions to help support your recovery. These may include: ice or heat application guidance, activity modifications or posture recommendations, simple home exercises or stretches.

Our goal is to ensure you feel comfortable, informed, and supported every step of the way.

Your Second Visit and Treatment Plan

At the end of your first visit, you can schedule a convenient time for your follow-up appointment. 

Most patients are scheduled within 1–2 days to ensure timely care.

During this second visit, you will receive:

A complete review of your examination findings

Chiropractic adjustment – head to toe, based on findings

A discussion of your personalized treatment options

Answers to your questions about care and recovery

We also invite all new patients to attend our New Patient Orientation, an engaging and informative session where you’ll learn more about chiropractic, wellness strategies, and your individualized treatment plan.
